It is for: ✅ Seniors living at home or in a retirement or assisted living setting ✅ Individuals managing chronic or complex health conditions ✅ Clients who require ongoing clinical oversight, not just one-time visits ✅ Families seeking professional guidance, advocacy, and coordination ✅ Clients who value having a dedicated Registered Nurse to navigate the healthcare system We offer 3 level of nurse-led care management to match the level of support required. 🟢 Essential Care Management, $650/month Foundational oversight and guidance Up to 2 hours of Registered Nurse support per month Ongoing care coordination and clinical oversight Care plan review and updates Communication with care providers and facility staff (with consent) Advocacy to support safety, preferences, and continuity of care 🟠 Enhanced Care Management, $950/month Increased support for evolving or more complex needs Up to 4 hours of Registered Nurse support per month Proactive care coordination and follow-up Appointment navigation and communication with healthcare providers Regular updates to family and care team Expanded advocacy and decision support 🔴 Concierge Care Management, $1,850/month Comprehensive, high-touch clinical oversight Up to 8 hours of Registered Nurse support per month Hands-on coordination across providers and services Direct liaison with physicians, facilities, and specialists Priority access and responsiveness Crisis navigation and ongoing care stabilization Additional Support Additional nursing time is available beyond included hours Provided at a preferred rate and only with your agreement Services remain flexible and adapt as your needs evolve How the Service Works ✔️ Monthly, ongoing support tailored to your needs ✔️ Services provided in-home, by phone, or virtually ✔️ Care plan reviewed and adjusted as your situation evolves ✔️ Unused monthly hours do not carry forward Service Adjustments & Clinical Travel 1. Care management plans are based on anticipated needs and may be adjusted. 2. Cancellation Policy
Vital Life Nursing Inc. – Refund & Rescheduling Policy No Refunds – All Sales Are Final Vital Life Nursing Inc. (“the Company”) maintains a strict no-refund policy for all prepaid services. By purchasing any service, digital product, online event, or course, you acknowledge and agree to the following: No refunds will be issued under any circumstances for prepaid, missed, cancelled, or completed services. All sales are final, and the Company does not provide money-back guarantees. This policy applies to private consultations, case management services, caregiver coordination, digital materials, online courses, workshops, and events. Rescheduling Policy: To reschedule a booked appointment, clients must contact the Company at least 48 hours prior to the scheduled session time. Rescheduling is subject to provider availability and cannot be guaranteed. Short-notice rescheduling for nursing services may be offered depending on provider availability; however, this is not guaranteed and remains at the discretion of Vital Life Nursing Inc. Completed sessions cannot be rescheduled or refunded. A session is considered completed once the client and provider connect online or in person at the scheduled time. Group sessions, classes, and workshops are non-refundable and non-reschedulable. Additional Terms Clients are encouraged to review the full Terms and Conditions and Service Agreement for more details, including expectations, professional boundaries, and provider responsibilities. By purchasing any service, you confirm that you have read, understood, and agree to these terms.
