Service Description
Initial Nursing Assessment is required for all clients who begin services with Vital Life Nursing, including: 1️⃣ Clients enrolling in Care Management 2️⃣ Clients receiving Caregiver Support Services 3️⃣ Independent clients requesting a standalone professional assessment by a Registered Nurse What the Initial Nursing Assessment Includes Our Registered Nurse completes a comprehensive, holistic assessment, which may include: ✅ Physical and mental health review ✅ Medication and current treatment review ✅ Post-hospital or post-surgery care planning ✅ Functional and mobility assessment ✅ Risk screening (e.g., falls, skin integrity, cognition) ✅ Social, emotional, and environmental needs assessment ✅ Chronic illness management support ✅ Assessment of conditions such as diabetes, heart disease, dementia, or cancer requiring ongoing oversight ✅ Advance care planning and review of client preferences Your Personalized Care Plan Following the assessment, the RN develops an individualized Care Plan that outlines: ⭐ Your current health needs and personal goals ⭐ Recommended nursing interventions ⭐ Required equipment or resources ⭐ Coordination and communication with your healthcare providers (with consent) ⭐ Goals of care and expected outcomes ⭐ Recommended frequency and type of nursing visits In-Home Nursing Visits A nursing home visit is a scheduled appointment provided in the comfort of your own home or residence to support health, safety, and quality of life. These visits may include: ✅ Health monitoring (vital signs, physical and mental status, early signs of change) ✅ Medication support and monitoring (administration oversight, side-effect monitoring) ✅ Wound and skin care, including prevention of skin breakdown ✅ Catheter care, including insertion, removal, and bladder flushing (within scope) ✅ Pain and comfort support, including cupping therapy to support circulation and pain relief ✅ Emotional and social support, addressing well-being, concerns, and reassurance Who This Service Is Ideal For 📍 Individuals transitioning home from hospital or surgery 📍 Clients managing chronic or complex health conditions 📍 Seniors requiring nursing support at home or in a retirement residence 📍 Individuals seeking ongoing, nurse-led oversight and advocacy 📍 BC residents and visitors to BC, Canada.
Cancellation Policy
Vital Life Nursing Inc. – Refund & Rescheduling Policy No Refunds – All Sales Are Final Vital Life Nursing Inc. (“the Company”) maintains a strict no-refund policy for all prepaid services. By purchasing any service, digital product, online event, or course, you acknowledge and agree to the following: No refunds will be issued under any circumstances for prepaid, missed, cancelled, or completed services. All sales are final, and the Company does not provide money-back guarantees. This policy applies to private consultations, case management services, caregiver coordination, digital materials, online courses, workshops, and events. Rescheduling Policy: To reschedule a booked appointment, clients must contact the Company at least 48 hours prior to the scheduled session time. Rescheduling is subject to provider availability and cannot be guaranteed. Short-notice rescheduling for nursing services may be offered depending on provider availability; however, this is not guaranteed and remains at the discretion of Vital Life Nursing Inc. Completed sessions cannot be rescheduled or refunded. A session is considered completed once the client and provider connect online or in person at the scheduled time. Group sessions, classes, and workshops are non-refundable and non-reschedulable.
